Will NOT run all programs!
I can't figure out exactly what the difference is, but if you try to run an LCD or similar off of this, it won't work. I can plug into a genuine Arduino Uno and it works perfect all day long. Use one of these, it communicates and downloads the code with no errors at all - so it SHOULD work. But it won't.
In short, some programs will work, others won't and will have you scratching your head. If you're doing simple stuff, no problem. If you're like me and using LCDs, WS2812b (need insanely accurate PWM timing), touchscreens, building an oscilloscope, etc - get a real one. That being said, $3 for a knock off Uno and it does almost as much...can't beat that with a stick. Just take it easy on these things, and don't expect it to do your taxes. As said above, you may have to load the coms drivers, but if you start having "this SHOULD work" problems, try it on a genuine board before you go reinventing the wheel.

Using this with an LCD touch screen or similar? Just buy an original Arduino product. Doing blink or something like that? This is great for that.
When doing things that need really good timing, fast memory access or whatever (like running WS2812 LEDs, using an SPI display, etc) do NOT go cheap, just buy the original product. But if you're just getting into this, running a servo or some little project with homegrown code - you can't beat a couple of bucks shipped to your house. You'll need to download the drivers for the serial, it's not the same as an original - but for probably 80% of the projects you would be doing with this in the Arduino IDE, it will work just fine.
